You can search for all incident reports in the database by the process steps where incident was discovered, type of incident, treatment modality, treatment technique, or a combination of these parameters.
  Incident Parameters
The radiation therapy modality used or planning to be used at the time of the incident.
Treatment Modality
The point in the workflow where the incident was initially discovered.
Where incident discovered
Actual event or good catch.
Incident type
The radiation therapy technique used or planning to be used at the time of the incident.
Treatment technique
Search for the incidents that were reported on or after this date.
Report Date: From
Search for the incidents that were reported on or before this date. This field can be left blank if you want to view the incidents reported on a single date selected in the "From" field above.
Report Date: To
  Analysis Parameters
A very brief title phrase describing the incident.
Summary title
The type of error that occurred or potentially could occur.
Error Type
The point in the workflow where the incident orginiated.
Where orginated
Dosimetry severity
Was the event reportable to the relevant regulatory agency (state, NRC)?
Reportable event
For potential incidents (good catches) assume a worst-case scenario of the incident were actually to occur.
Medical severity
The method used for assessing the severity of harm.
Severity assessment type
The time after the incident at which the severity of harm was observed or at which it is estimated to occur.
